Sub-Acute Care and Skilled Nursing

Mission Health managed Communities tailor sub-acute care and skilled services to each resident's unique needs and medical requirements. These services feature today's technologies and care performed by healthcare professionals.

  • Individual Care Plans - Comprehensive, individual care plans are created in tandem with your physician and the Mission Health care team to outline the strategy of care during your stay at our Community.
  • 24-Hour Skilled Nursing - All Mission Health Communities are staffed by Registered Nurses, Nursing Aides, Certified Nursing Assistants or Licensed Practical Nurses who work closely with your physician or the care team assigned to your overall health.
  • Intravenous (IV) Therapy - Mission Health IV therapy delivers needed fluids, specialty medications and nutritional supplements to our residents.
  • Wound Care Management - Mission Health care teams include healthcare practitioners with experience in wound care management. Our wound care nurses are trained professionals whose goal is to promote healing of wounds such as: abrasions, lacerations, contusions, avulsions, diabetic foot ulcers, venous and arterial ulceration, and traumatic and surgical wounds.
  • Tracheostomy Care - Our skilled nurses are trained in tracheostomy care and the maintenance of artificial airways . Mission Health tracheostomy care programs include assessment and evaluation, suctioning, stoma care, dressing changes, changing inner cannula and other essential services.
  • Diabetes Care - Mission Health care providers can help residents manage the complex nutritional, medical and treatment requirements associated with Type 1 and Type 2 diabetes.
  • Radiology - Our health and rehabilitation Communities are equipped with access to radiology equipment and radiologists to provide medical imaging that is crucial to residents progress and care strategy.
  • Laboratory & Pharmacy Services - Laboratory and pharmacy services play an important role in recovery and rehabilitation. Mission Health Communities provide access to both laboratory and pharmacy services in support of our commitment to an optimal rehab experience.
  • Bariatric Therapies - Mission Health Communities offer bariatric services for patients who require inpatient rehabilitation or care for a range of neurological, orthopedic, cardiac and complex medical diagnoses. Our care teams interact directly with referring hospitals to make sure the appropriate equipment and resources are in place to facilitate personal comfort and functioning.
  • Advanced Dysphagia Treatment - Our team of skilled nurses can perform dysphagia treatment and assist residents who struggle in swallowing medication or food.
  • Behavioral & Psychiatric Health Services - Mission Health Communities are equipped to assist individuals struggling with behavioral and mental health concerns. In some cases, these services are offered on-site but are delivered by a third-party provider.
  • Pain Management - At Mission Health, the physical comfort of our residents is a priority. Our health and rehabilitation Communities offer pain management services with the goal of providing relief and assistance in coping with chronic and severe pain.
  • Dementia Care - Many Mission Health Communities have secured Memory Care Units that specialize in assistance for residents who have been diagnosed with Alzheimer's disease, dementia or other memory deficiencies. Mission Health Secured Care Units strive to help each resident to function at their most optimal level of ability, while team members provide care, assistance and supervision.
  • Nutritional Services - In addition to encouraging our residents to participate in meal selection, our nutritional service menus are developed to comply with both personal preferences and individual dietary requirements/restrictions. All Mission Health Communities offer group dining facilities as well as private dining spaces for family celebrations.
  • Additional Services - All Communities are prepared to facilitate resident access to additional clinical care resources including eye care, podiatry and dental services.
